RENEWAL REQUIREMENTS
Your membership in any Medicaid program will need to be renewed
on a regular basis, typically each year. You will be
contacted by mail when it is time for you to renew your
enrollment. It is important for you to respond to all mail
you receive regarding your coverage. If you re-enroll on time, you
will not experience any break in program services. If you do not
re-enroll you may have a break in coverage or lose your
coverage.
If you have a change of address or phone number, it is
critical for you to inform your DFR case manager or contact your local Division of
Family Resources so that you will receive inportant
notifications.
