Reporting a Change
It is very important to let Medicaid know of changes in order to
keep your coverage and to have access to important information.
Common changes are reporting an address change or a gain or loss of
income.
REPORTING A CHANGE OF ADDRESS
It is very important that you keep your address
updated. You can call or visit your caseworker or local DFR
office to change your address. Without a current address
on file we will not be able to reach you with important information
about your Medicaid benefits. The numbers for the health
plans are listed below:
REPORTING A CHANGE IN INCOME
It is very important for you to notify your caseworker or local
DFR Office if you have a change in income. This may
impact your eligibility. However, it is important to be accurate in
reporting your income because failure to do so could cause you to
be ineligible for an extended period of time and you may be
required to reimburse Medicaid for benefits paid on your
behalf.
