Care Select Redesign
Effective October 1, 2010, the Care Select program will be
changing to a disease management program. The changes will focus
the Care Select program towards its original goal of disease
management for the following chronic conditions: asthma, diabetes,
congestive heart failure or coronary heart disease, hypertension,
chronic kidney disease, severe mental illness (SMI), serious
emotional disturbance (SED), and depression.
Eligible members with one of the above listed conditions will
automatically be enrolled in the Care Select disease management
program. Members who choose to opt out of the Care
Select Program, members that do not have one of the above
conditions and Home and Community Based Services (HCBS) waiver
members will be enrolled in Traditional Medicaid. HCBS waiver
individuals receive case management services through the
waiver.
