Important Hoosier care connect update

Beginning April 1, 2017, MDwise will no longer be a Hoosier Care Connect health plan. If MDwise is your current plan, you will need to select another health plan to coordinate your care in Hoosier Care Connect.

You will have until March 15th, 2017, to call the Hoosier Care Connect program at 1-866-963-7383 and choose a new health plan.  If you do not choose a health plan by March 15th, 2017, we will choose a plan for you.

You still have Medicaid and will receive full Medicaid benefits as a member of Hoosier Care Connect. You will continue to receive any services that have been authorized for up to 30 days or until the end of the authorization or whichever comes first.

What is a health plan?

A health plan is a group of doctors, pharmacies and hospitals that work together to help you get the health services you need. You may choose one of the following two health plans: Anthem or Managed Health Services (MHS). All plans offer the same Medicaid services but may work with different doctors or hospitals and may offer special programs that you would like. A Health Plan Summary sheet with more information about the health plans is included with this notice.

Action Needed

  • Review the Health Plan Summary sheet to learn more about the different health plans (Anthem and MHS) and decide which is best for you. Call the Hoosier Care Connect Helpline at 1-866-963-7383 to learn more and to see which plans your doctor's work with. You should consider which plan includes your primary medical provider and any specialists you see.
  • Call the Hoosier Care Connect Helpline at 1-866-963-7383 to choose your health plan. It is better for you to choose so you can get the care you need from your own doctor who knows your medical history.
  • Ask a family member or friend for help if needed.
  • Choose a health plan by March 15, 2017, or one will be chosen for you.
  • If you are unhappy with your assigned plan and wish to change, you will have an opportunity to change MCEs until August 1, 2017.
  • Continue to use your existing Medicaid card during this transition.
  • In addition, you will need to contact your new MCE and inquire about in network pharmacies.
  • Call the Hoosier Care Connect Helpline at 1-866-963-7383. The helpline will provide you unbiased information about your options.
