Beginning January 1, 2011, a Preferred Diabetic Supply List (PDSL) will be implemented.

What does this mean for me?

You will need to start using one of the blood glucose monitors listed in the table below. If you have a blood glucose monitor that is not on the list below, you will need to speak with your doctor to get a new prescription for one of the blood glucose monitors and test strips listed below.  New blood glucose monitors and test strips will be provided at no cost to you.  If you already have one of the blood glucose monitors listed below, you do not need to do anything.

What if I need a monitor not on this list?

If you have a medical condition that prevents you from using one of the blood glucose monitors listed above, you or your pharmacy will need to speak with your doctor and ask him to request a prior authorization for a different blood glucose monitor and test strips.

What is not changing?

You may continue to use the same provider(s) for your diabetic supplies as you do now, and you will continue to receive these products at no charge to you.
